Icicles and multistrike

Question:

If your frostbolt multistrikes 2x, do you generate 1 icicle from the sum of the hits (FB dmg normal + multistrike A dmg + multistrike B dmg) or do you generate 3 icicles for each hit?

When you damage enemies with Frostbolt and Frostfire Bolt, and their multistrikes, 16% of the damage done is stored as an Icicle with you, for 30 sec. Also increases the damage of your Water Elemental's Waterbolt by 16%.
What this also means is that Icicles cannot themselves Multistrike, or it would double dip.

You generate 3 icicle
For example: Frostbolt's damage is 100 and you have 40% mastery, then you will generate:
1 icicle with 0,4*100=40 damage from the base spell and 2 icicles from the double multistrikes, each dealing 0,4(0,3*100)=12 damage
